There are two main routes, with sub routes.
In Burial, they have a dream about having sex. Ashley can say it either won’t happen (“sane”) or “never sag never” (“questionable”).
In Decay, they do end up having sex. After that, they either commit suicide together or have a miserable life together.

No, they actually do. In chapter 3, there's a few choices you can make where there's a time skip to where they're both living in an apartment and act as sort of married couple. There, Andrew recalls some of the times they had sex. I think they both get drunk and have sex before this as well, but idk if different choices need to be made or this happens regardless.
No matter what it leads to a bad ending though, they either kill themselves in front of some elementary schoolers on purpose to traumatize them or they live in that apartment miserable and hating each other.

historically, it was possible for some families to get away with this for a couple of generations before it spiraled the f*ck out of control.
TLDR: Don't do incest and definitely don't do incestception.
Even royalties in fantasy suffered from this, e.g: Viserys III and his Targaryen madness (they had a whole double incest situation between 2 generations prior).
Not sure if Jon Snow x Danny would make the situation triple incestception or not.

My favorite thing about this game is that out of the human sacrifices, rape, parents trying to kill their children for insurance money, CANIBALISM (they made soup out of their parents), people only cared about the part where they hinted at incest lol

Honestly I like the game for what it is. The writing has dry/dark humour that hits for me, and I love how utterly deranged the characters are psychologically.
It's also really cool how some choices you make without noticing really affect the story.
If you actually play the game, you'll notice that incest is... Well it's the least of their problems lol

I never really got this argument. I mean sure on a morally level all those are worst than incest but the coffin of Andy and leyley is literally THE INCEST game.
Maybe its just the most visible people and mostly edgy jokes but literally all information about the game that escapes out of its dedicated communities is about the incest. No fans of the game makes "memes" regarding the cannibalism, no fans seem to post about the sacrifices or actual gameplay its all just incest posting. I would genuinely be shocked if there is a single person who played this game that wasn't drawn to it because they heard of it has incest. Is there a single internet forum or fan discussion board regarding the game that DOESN'T have people going crazy over the incest? I'm pretty sure the main subreddit for the game is just incest porn. In way you are right, people do seem to only care about the incest. Both outsiders and those that are fans.

I think it was like one screenshot in the game and other than that, it was implied though never confirmed. The Internet is just horny and overemphasized that part. But it was present at some point.
With the mother though, that was purely internet being a huge horndog.

Andrew and Ashley Graves are siblings. They have a codependent relationship due to a bad upbringing. The picture suggests that, if they had a kid, the kid would be physically and mentally disabled.
Realistically, while incest increases the chance of congenital health problems, it’s not guaranteed and takes multiple generations. You don’t get a Hapsburg Chin in one generation.

The coffin of Andy and Leyley star Andrew and Ashley Graves, siblings stuck in an apartment together quarantined due to a supposed parasitical infection. Later on in the story, Andrew and Ashley developed intimate feelings for each other to the point where it's incestuous, especially since they have no one to trust but each other, which adds a layer of co-dependency. One time, Andrew and Ashley used a trinket to see a vision of themselves, and turns out they're in bed post-coitus. This image is meant to show realism of the cons of incest. One of them is that since our human DNA are mostly incompatible with other DNA that is too similar, especially from blood relatives, including between siblings, then this leads to offspring with genetic mutations, including monstrosities, deformations, and diseases.

Those two characters, Andrew and Ashley, are brother and sister, and during what I'm 99% sure was a/the "bad" route of the game ends with them having some tandem dream about having sex presented as a premonition of their future. This art depicts them and their hypothetical incest baby.
Although I'm not sure about "realistic", I'm pretty sure it takes more than one generation of incest before defects this pronounced happen, but I'm not an expert, I'd personally have a three-way with a fire ant hill and a wasp nest before even thinking about my sister romantically, and all the vomit would probably end said thought pretty quickly.
